Important note for successful booking of COVID-19 vaccine:

On the day of vaccination, you should
- Bring the following documents: (i) original identification documents ; (ii) priority group supporting document (if applicable); and (iii) proof of successful booking of the booking system.
- If you are under 18 years old, please download and complete the consent for vaccination by parent/ guardian and bring it along to the vaccination venue.
- For children aged between 3 to 11 years old, an accompanying adult (e.g. adult relative, domestic helper or school teacher if the child receives vaccination via group arrangements by schools) is required to be present physically at the venue on the day of vaccination. If the identity document of the child (e.g. birth certificate) does not bear a photo, please remember to bring the child’s latest student’s handbook or school record bearing the child’s name, date of birth and photograph, or other identity document with photo to facilitate the healthcare staff to verify the identity of the child. For children less than 3 years old, parents should provide the original copy of the identity document (e.g. birth certificate) of the child, the signed consent form and accompanied by the parent / guardian for vaccination. Should the identity document of the child does not bear a photo and the child does not have a student's handbook or other identity document with photo, discretion will be exercised to verify the identity of the child by other means in light of the circumstances. If the vaccination is arranged by the school / centre in a group, the accompanying teacher or staff should bring the aforementioned required documents and be responsible in clearly indicating the identity of each child.

- Please wear clothes that the upper arm can be exposed easily for vaccination.
- For children and adolescents receiving the mRNA vaccine, the vaccine is suggested to be administered in the muscle of the mid-anterolateral thigh. They should wear loose-fitting clothing for ease of vaccination.

Please be aware of the following situations:
- If you are feeling unwell on the vaccination day, such as having fever or cough, please do not visit the vaccination venue and seek medical attention promptly.
- If you are in doubt on the suitability for vaccination due to your background medical illnesses, please discuss with your doctor.
- If you have chronic illnesses, please consult your doctor for suitability of vaccination in advance before the appointment date.

- Eligible citizens aged 6 months or above who are still not completed initial dose(s) can receive their initial dose(s) for free. Eligible citizens belonging to high-risk priority groups, regardless of the number of dose(s) received in the past, can receive booster for free at least 180 days after the last dose or COVID-19 infection (whichever is later). - Citizens going to the vaccination venues should also comply with the visitor infection prevention measures of the individual venue.
